The Case Manager, PCICM provides comprehensive, strength-based, and trauma-informed case management to a small caseload of high-needs refugee and immigrant clients enrolled in the PC-ICM (Preferred Communities Intensive Case Management) program contracted through Catholic Charities USA (CCUSA). This position ensures that each client receives individualized, culturally appropriate services aimed at promoting self-sufficiency, stability, and community integration. The Case Manager conducts assessments, develops and monitors individualized service plans, provides direct support, makes referrals, and advocates for clients’ access to services including health, housing, employment, ECBO’s (Ethnic Community-Based Organizations), education, and mental health resources.
