This position works a 32-hour reduced work week while being paid for 40 hours. Avivo’s Ending Homelessness division believes safe housing for all is a social justice issue and a human right. The PATH program supports individuals with serious mental illness (SMI) and/or co-occurring substance use disorders through a continuum of support toward housing and positive behavioral health outcomes. As a Case Manager, you will provide direct, field-based street outreach, engagement, and case management services to individuals experiencing homelessness. This role emphasizes person-centered, trauma-informed, and culturally responsive support aligned with Avivo’s commitment to dignity and connection. You’ll meet participants where they are—streets, encampments, shelters, and other nontraditional settings—helping them navigate housing and essential services.
