About The Position

Under the supervision of the Clinical Coordinator, the Case Manager provides direct client support and care coordination for individuals served by the Mobile Medication Unit (MMU). The Case Manager facilitates admissions, discharges, recovery planning, and linkage to community-based resources. This role focuses on removing barriers to care, promoting engagement, and supporting client success. The Case Manager travels with the MMU to designated community locations.

Requirements

  • High School Diploma or equivalent required; Bachelor's Degree preferred.
  • Two (2) years of experience in case management, substance use disorder, or behavioral health setting required.
  • CASAC, CASAC-T, required
  • Knowledge of community resources, MOUD, and recovery-oriented systems of care.
  • Strong interpersonal, communication, and organizational skills.
  • Proficiency with Microsoft Office, Electronic Health Record, and data tracking systems.
  • Ability to engage clients from diverse backgrounds with empathy and professionalism.
  • Must be able to work a flexible schedule including early mornings and weekends.

  • Bilingual (English/Spanish) preferred.

Responsibilities

  • Conduct intake screenings and complete admission documentation for new clients.
  • Develop individualized service plans and discharge plans in collaboration with the treatment team.
  • Provide case management services including linkage to medical, behavioral health, housing, and social support.
  • Facilitate coordination between the client, Clinical Coordinator, nursing staff, and community partners.
  • Monitor client attendance, engagement, progress toward treatment goals and adherence to MOUD.
  • Conduct discharge planning to ensure continuity of care and referrals for MOUD and recovery supports.
  • Maintain accurate, timely documentation in the Electronic Health Record (EHR).
  • Provide client advocacy and support using motivational interviewing and harm reduction principles.
  • Assist clients with transportation, benefits enrollment, and other practical needs related to treatment access.
  • Participate in multidisciplinary meetings and case conferences.
  • Collaborate with the Clinical Coordinator to track data, report outcomes, and support quality improvement initiatives.
  • Help manage client flow and ensure a supportive, nonjudgmental service environment.
  • Perform any additional duties, tasks, and responsibilities that may be assigned, as needed.
