About The Position

The Case Manager (Mental Health) at Children's Hospital of The King's Daughters is responsible for providing support and case management services to patients and families within the Mental Health Service Line Outpatient Program. This role involves collaborating with multidisciplinary teams to facilitate treatment goals, coordinating referrals, and ensuring continuity of care. The Case Manager will work directly with patients and families, offering resources and support while maintaining accurate records of local mental health resources.

Requirements

  • Master's degree in Social Work, Counseling, or related field from an accredited college or university.
  • 1-2 years of related work experience preferred.
  • Coursework in case management, advocacy, and family systems preferred.
  • Preferred experience working in healthcare environments.
  • Preferred experience with implementing evidence-based treatments with children and families.

Responsibilities

  • Assesses the biopsychosocial needs of patients and caregivers.
  • Processes new assigned cases through outpatient services team.
  • Collaborates with patients and interdisciplinary medical teams to develop service plans.
  • Monitors the receipt of services to reassess needs and ensure timely care.
  • Documents relevant information in medical records.
  • Coordinates patient care conferences regarding case management issues.
  • Establishes therapeutic relationships with patients and caregivers.
  • Provides counseling and mental health interventions under supervision.
  • Works with community resources to meet patient service needs.
  • Participates in interdisciplinary meetings and trainings.
  • Develops service plans with patients and families to implement appropriate services.
  • Provides ongoing supportive and case management functions.
