About The Position

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Mental Health Professional (MHP) Lead who demonstrates this mission and wants to work for an organization that makes a difference in the community.

Requirements

  • Education: Bachelor’s degree in a human services field required
  • Experience: 1+ years of experience in behavioral health preferred, bilingual preferred
  • Physical: Navigation of stairs
  • No lifting restrictions.
  • Ability to provide services in clients’ homes.
  • Equipment: Computer including Microsoft Windows applications Copy Machine Telephone with voice mail system
  • Additional Requirements: The use of personal automobile, a valid driver’s license, and the minimum amount of liability insurance as defined by AID’s Personnel Policy
  • Drive self and clients in agency or personal vehicle.
  • Must acquire and maintain certifications in First Aid, CPR, Non-violent crisis intervention training, CEU’s and other relevant trainings
  • Overnight, Evening, Weekend and Holiday hours required
  • Providing coverage across multiple Supervised Program Facilities required
  • Covering non-scheduled shifts as needed on call
  • Must be able to drive a passenger vehicle
  • Must Maintain IM+CANS certification

Responsibilities

  • Complete Intake/IM+CANS and other required paperwork
  • Conduct new client intakes including IM+CANS and LOCUS Assessments for all new client intakes.
  • Maintain prescreening/intake logs.
  • Provide documentation of services in accordance with DHS and CARF.
  • Advocate for and maintain rights of clients that are served.
  • Assignment of clients to case managers’ caseloads in coordination with program manager(s).
  • Training new staff and completing BH Division New Employee Checklist
  • Models appropriate behavior and boundaries with teammates and clients
  • Acts as an extension of management and assist manager as needed
  • Work scheduled hours, be flexible to meet client and program needs, including cross training in other programs as assigned by supervisor.
  • Complete clinical assessments that meet the requirements of program funders and applicable administrative rules
  • Complete all required documentation in a timely manner
  • Acquire and maintain certification in mandated trainings including First Aid, Adult CPR, & CPI
  • Employee promotes awareness and respect for the diversity of our clients, employees, families and other stakeholders.
  • Schedule new client intakes within the timeframe set by agency policy.
  • Complete Consent for Assessment, Client Rights Form, agency paperwork, IM+CANS and LOCUS assessment in a timely fashion using Cx360.
  • Must meet minimum productivity standards (MRO) monthly
