We are looking for a Case Manager Mental Health Professional IM+CANS Specialist for both Central Kane County (Batavia, St. Charles, Geneva area) and DuPage County, IL who demonstrates this mission and wants to work for an organization that makes a difference in the community. The CM MHP IM+CANS Specialist is responsible for ensuring assessments remain updated every six months for all adult clients who are diagnosed with persistent mental illness and co-occurring substance use disorders, seeking services in BH Outpatient/PSR and Supported Programs. The CM MHP IM+CANS specialist is expected to work with a team of service providers within the agency and throughout the social service community to ensure that all needs of clients are met. Provision of assessment, counseling, skills training and case management services to a caseload of clients who are diagnosed with persistent mental illness disorders. Demonstrate understanding of chronic illnesses frequently experienced by those diagnosed with severe mental illness. Demonstrate knowledge of assessments and treatment planning. Ability to effectively engage client in IM+CANS development. Track outcomes, be highly organized, and have strong clinical writing skills. This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment.
