Responsibilities include coordinating treatment plans, conducting educational groups, and ensuring continuity of care post-release. The role requires close collaboration with medical, security, and administrative staff to deliver integrated, effective support for inmates undergoing MAT.

Requirements

  • Bachelor's degree in substance use, social work, psychology, or related field from an accredited college or university or nursing license; LPN
  • One (1) year experience required.
  • Possess and maintains current CPR certification.

Nice To Haves

  • Mental Health, Substance Use Disorders, Medication Assisted Treatment, or Case Management in a corrections environment preferred.

Responsibilities

  • Assists the Director of Recovery and Reentry Services (DRRS) in managing the case management needs of individuals in MAT treatment, ensuring coordinated services integrated with security, medical, and administration.
  • Conducts educational groups, provides routine case management services, and ensures appropriate documentation and active involvement in treatment team meetings.
  • Provides discharge planning services to support MAT continuation post-release, ensuring all services are documented in the medical record as per protocol.
  • Attends clinical supervision, required trainings, and relevant meetings, assists the DRRS with data collection and entry for all MAT visits, and prepares statistics for third-party agencies.
  • Networks with administration, correctional officers, medical, and outside providers to maintain positive working relationships, abides by security regulations, protects the confidentiality of inmate information, and adheres to HIPAA Regulations.
