Position Summary The Case Manager provides program support and service coordination for people served in our Life Enrichment Day Support Services Program. Working with persons with disabilities, the Case Manager provides community integration and inclusion through recreation, skill-building, education, and job exploration, including positive approaches to learning, systematic instruction, behavioral intervention, case management, and self-care needs. This position also provides transportation services for individuals as needed. The Case Manager position is an excellent opportunity if you are passionate about working with persons with various disabilities. This role is a great pathway opportunity for persons aspiring for careers in Social Work, Human Services, and/or Health Care. This position splits time between two programs located in Coon Rapids and Mounds View. The hours for this position are Monday - Friday from 7a-3p.
