About The Position

Case Manager Lead | CFSS Harry Meyering Center supports people with developmental and intellectual disabilities in their homes and community. Do you have great organizational skills and the ability to communicate social services to individuals with disabilities? The Facilitator Lead is responsible for leading a team that implements Community First Services and Supports (CFSS) Consultation Services, a new program being developed in Minnesota. In this role, the Facilitator Lead supervises Navigators and Facilitators, providing guidance on developing plans, conducting orientation, and offering ongoing support for people using CFSS. The Facilitator Lead supervises, hires, and trains staff, and assists them in completing tasks related to CFSS, including person-centered planning. The Facilitator Lead also maintains a caseload and supports the development of CFSS Service Delivery Plans, which are an integral component of the program. Based on assessments, participants identify their needs and goals, and the Facilitator Lead helps facilitate the creation of plans to meet those needs. Using principles of self-direction and person-centered planning, the Facilitator Lead provides guidance and assistance as needed to ensure plans are effectively implemented. Schedule: Monday – Friday, 8:00 am – 4:30 pm. Occasional nights or weekends may be needed. Position type: Hybrid Location : This will be a Hybrid position based at our Mankato, MN office. Wage : $26.50 per hour.

Requirements

  • Must be 18 years of age or older.
  • Must pass the MN Department of Human Services Background Study.
  • Bachelor’s degree in a field related to human services or equivalent experience.
  • OR one of the following: One year of full-time experience providing direct services to people with disabilities or individuals over age 65.
  • Experience coordinating or directing services for people with disabilities or individuals over age 65, including self-directed services.
  • Experience coordinating their own services.
  • Completion of a DHS-approved training module of at least six hours on person-centered planning, the Vulnerable Adult Act, and the Maltreatment of Minors Act.
  • Valid driver’s license and clean driving record verified by our insurance carrier.
  • Excellent verbal and written communication skills, with the ability to interact effectively with individuals, support teams, case managers, family members, and colleagues.
  • Self-starter with strong interpersonal and problem-solving skills, capable of responding to emergencies.
  • Strong organizational skills, ability to handle multiple competing priorities.
  • Proficiency in technology programs (e.g., Outlook, Excel, Word, Adobe, PowerPoint), with a willingness to learn new systems.
  • Ability to work collaboratively with colleagues and independently to fulfill job responsibilities.
  • Strong documentation skills and familiarity with systems, routines, and programming.

  • Preferred qualification of fluent in understanding and speaking multiple languages other than English.

Responsibilities

  • Provides mentorship, training, supervision, and assistance to individuals and team members in a genuine, person-centered, and respectful manner while maintaining professional boundaries
  • Provides direction to the team, including scheduling, coordination of orientation, staff meetings, and performance management. Including annual reviews.
  • Communicates changes to the program, information, and other policies to the team.
  • Provides quality and compliance oversight on budgets, service delivery plans, and customer service.
  • Be a resource to the team to answer questions for team members.
  • Assist with customer service, including escalated situations that require sensitivity.
  • Demonstrates exemplary supervision skills, problem-solving, and responsible work habits.
  • Assist with caseload management if a team member is out for an extended period.
  • Maintain caseload of people using CFSS Services.
  • Use various person-centered planning methods to encourage participants to develop an individualized plan that will meet their needs.
  • Be independent in understanding, communicating, and ensuring that participants understand the rules and regulations for the CFSS program
  • Implement the services required under the CFSS Consultation Services contract, including meeting with individuals to explain the program, developing and monitoring plans, and providing guidance and training to individuals and the responsible party. This may include in-person meetings, online meetings, and webinar-style meetings.
  • Meet with participants as required by CFSS requirements.
  • Write in a manner that is easy for the participant to understand.
  • Review options for revising or changing the plan for participants.
  • Assist the participant in identifying health and safety risks, including identifying appropriate staffing, PERS, other goods and services, a backup emergency plan, and worker training and development.
  • Assist participants using the budget model to understand all employer responsibilities, including tracking employee competencies, evaluations, training, and other requirements.
  • Assessing environmental concerns or hazards and identifying when a change in condition may necessitate a reassessment.
  • Reviews the plan to ensure that the plan: Includes outcomes that meet the needs of the individual based on the assessed needs
  • Reviews the plan to ensure that the activities, goods, and services are within the scope of CFSS services.
  • Communicate with people using services, their guardians, parents, case managers, and other care providers in a professional manner, in-person, verbally, and in writing
  • Handle sensitive information with discretion and maintain privacy.
  • Promote diversity, equity, and inclusion.
  • Ensure HIPAA compliance in all communications, both internal and external.
  • Adhere to health and safety policies and procedures.
  • Participate in professional development opportunities.
  • Perform other duties as assigned.
