About The Position

Performs advanced case management work. Work involves coordinating case management and program activities; developing program goals, objectives, and procedures; and evaluating outcome measures for the program. Works under minimal supervision with extensive latitude for the use of initiative and independent judgment.

Requirements

  • Graduation from an accredited senior high school or equivalent or GED.
  • Four years full-time, wage-earning criminal justice, social services, human services, case processing, or case management experience.
  • Fifteen semester hours from a college or university accredited by an organization recognized by the Council for Higher Education accreditation (CHEA) or by the United States Department of Education (USDE) may be substituted for each six months of experience.
  • Knowledge of case management principles, practices, standards, and methods.
  • Knowledge of offender treatment principles, methods, techniques, and practices in correctional rehabilitation programs.
  • Knowledge of classroom dynamics.
  • Knowledge of applicable state and federal laws, rules, regulations, and statutes.
  • Skill to communicate ideas and instructions clearly and concisely.
  • Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.
  • Skill to interpret and apply rules, regulations, policies, and procedures.
  • Skill in problem-solving techniques.
  • Skill to prepare and maintain accurate records, files, and reports.
  • Skill to plan work in order to meet established guidelines.
  • Skill to review technical data and prepare technical reports.
  • Skill in interviewing.
  • Skill to assess needs and coordinate ongoing or transitional services.
  • Skill to utilize and identify available community resources.

  • Knowledge of agency and departmental organizational structure, policies, procedures, rules, and regulations preferred.

Responsibilities

  • Coordinates case management and program activities
  • Coordinates maintenance of documentation on case management and rehabilitation programs
  • Assists in program monitoring and evaluation.
  • Develops, presents, and conducts educational and programmatic groups in accordance with agency curriculum and standards
  • Assists with development and implementation of program interventions including curriculum
  • Documents and reviews case management and progress records.
  • Documents and reviews case records for group and individual meetings or sessions
  • Evaluates case documentation
  • Provides advanced case management services to those with higher need levels
  • Monitors peer support operations.
  • Identifies problem areas, service gaps, and areas that are barriers to services for inmates
  • Makes recommendations to produce a more effective program.
  • Conducts orientation sessions with incoming populations
  • Attends Unit Classification Committee (UCC) meetings, sanctions meetings, treatment team meetings, and participates in program completion transition meetings.
  • Performs a variety of marginal duties not listed, to be determined and assigned as needed.
