About The Position
A growing Medicaid home care agency in the Phoenix metro area is seeking a Case Manager – Intake to support referral coordination and intake for Arizona Medicaid members. This role focuses on managing MCO referrals, authorizations, and onboarding processes to ensure timely and compliant service initiation.
Requirements
- 2+ years of experience in Medicaid intake, case management, or healthcare coordination
- Familiarity with managed care workflows and authorization processes
- Strong organizational and communication skills
- Ability to manage high-volume referrals in a fast-paced environment
Nice To Haves
- Experience with Arizona Medicaid and AHCCCS programs
- Familiarity with MCOs such as UnitedHealthcare, Banner Health, or Mercy Care
- Background in home care, care coordination, or utilization management
Responsibilities
- Manage intake referrals from Medicaid managed care organizations
- Verify eligibility and approved service hours
- Coordinate authorizations and intake documentation
- Communicate with MCO case managers and internal care teams
- Support onboarding and start-of-care coordination
- Ensure services align with approved authorizations and care plans
- Maintain accurate records and compliance documentation
- Assist with resolving intake-related issues and delays
