About The Position

Catholic Charities Twin Cities is to serve those most in need and to advocate for justice in the community. Our programs for children, families, and adults serve more than 25,000 people every year. As the need for our services grows, Catholic Charities is working to adapt and respond with strategies that prevent poverty, meet basic needs in times of crisis, and create pathways to greater stability. The Homeless Elders Program provides in-person housing-focused case management to older adults experiencing homelessness with the goal of assisting them with obtaining and maintaining safe, stable housing. This position provides case management services in a residential setting at Trellis’ new affordable housing site in St. Paul, the Treehouse Apartments. The caseload will consist of 18 low income, formerly homeless older adults who will reside in the building (of the total 36 units). This position will also involve working with Ramsey County’s Coordinated Entry system to fill open units at the building, as well as Housing Support.

Requirements

  • Bachelor’s or master’s degree in social work, Psychology, Sociology, Human Services, or related field preferred. Or a bachelor’s degree with a major in any field and at least one (1) year of experience as a social worker/case manager/care coordinator in a public or private social services agency.
  • Pass DHS background check and meet their requirements for the provision of case management by meeting the social work standards under the MN Merit System.
  • Additional training, education, or licensure may be required per program need.
  • Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
  • Must have a valid driver’s license and proof of current insurance coverage if transporting clients.

Responsibilities

  • Provide comprehensive case management services on site, including: Assessment of needs Goal setting Connecting residents to resources and additional support services
  • Provide skill building to assist in housing stability
  • Responsible for documentation and some data entry, reviewing and ensuring the relevance, accuracy, and timeliness of data in client files.
  • Assist in maintaining statistical and other data in specific systems to support and facilitate required reporting.
  • May process invoices and other department-specific documents.
  • Enroll clients in Housing Support at intake and assist with management of Housing Support administration functions
  • Collaborate with housing project’s owner and management company to ensure quality service.
  • Collaborate with Ramsey County coordinated entry system and assist with screening potential residents.
  • Other duties as assigned
