About The Position

LifeLong Medical Care is recruiting to temporarily fill the position of an Associate Behavioral Health Clinician. (ACSW/ASW) position within the innovative Street Medicine program. Examples of our work can be seen in a variety of local and national news articles, particularly as it pertains to our COVID-19 response within the population of people experiencing homelessness in Alameda County. The ACSW/ASW works as part of an integrated, multidisciplinary team including a community health worker, registered nurse, and primary care providers to assess, treat, and support the holistic needs individuals living in encampments and on the streets experiencing homelessness. This position will be deployed in one of the five service areas in Northern Alameda County that LifeLong Street Medicine serves as part of its partnership with Alameda County Health Care for the Homeless. The ACSW/ASW works with patients and partner organizations to provide short-term intensive transitional case management until a patient has stabilized enough to transition to a conventional system of care (primary care and/or a community mental health clinic or Intensive Case Management program) or has been placed in a higher level of care. As a key member of this team, the ACSW/ASW is responsible for outreach and engagement, mental health assessment and intakes, care plan development, intensive short term case management of a 25-patient caseload, collaboration with licensed mental health clinicians and the larger medical and mental health system. The ASW/ACSW is also the team lead to conducting housing assessments and supporting patients to secure housing when matched to a housing resource. Ideally, the ACSW/ASW will be a trusted member of the community in which they will work. This relationship will enable the ACSW/ASW to serve as a liaison between health, social services, and housing resource providers. Following LifeLong COVID-19 safety protocols and exercising good judgment regarding risk mitigation are important for both staff and patient safety.

Requirements

  • Ability to work flexibly and collaboratively in both non-traditional, street and encampment settings and in a fast-paced primary care clinic.
  • Must be an independent self-starter, able to balance outreach, counseling, case management and administrative tasks.
  • Ability to work effectively with a multi-disciplinary team.
  • Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle, and experiential backgrounds.
  • Excellent verbal, written and interpersonal communication skills.
  • Computer literate in MS Office applications and willing to learn clinic systems.
  • Master’s degree in social work and be a registered ASW with California Board of Behavioral Sciences.
  • Possession of current driver’s license and insurance.
  • Knowledge and experience providing mental health services to high-risk populations.
  • Familiarity with evidence-based practices for treatment of depression and other mental disorders.
  • Experience in clinical case management and harm reduction.

Nice To Haves

  • Experience with homeless adults and/or adults with serious mental illness and substance use disorders.
  • Lived experience relative to working with people experiencing homelessness (e.g. formerly homeless, social or behavioral health services consumer, criminal justice system involvement, foster care involvement, close family-member of someone with these experiences).

Responsibilities

  • Participates with street health team members in conducting street outreach, intake and assessment for people living in homeless encampments.
  • Conducts behavioral health assessments and triage, creates client centered care plans and implements brief interventions and short-term counseling using appropriate modalities for patients with depression, anxiety, trauma, grief/loss, addiction etc.
  • Supports linkages and coordinated care with public and community based mental health and substance use treatment systems.
  • Provides intensive case management to a caseload of 25 clients, focused on linkage to health care, housing and benefits.
  • Assists with crisis intervention as needed and including assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
  • Follow up to crisis intervention as appropriate.
  • Participates in daily huddles, outreach and follow up planning and weekly case conference ensuring coordination of care with medical providers and other inter-disciplinary team members.
  • Maintains clinical and administrative records in accordance with contractual and billing requirements.
  • Consults with medical providers as needed.
  • Follows all State of California mandated reporting requirements.
