About The Position

Diakon is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Child, Family, and Community Ministries and Diakon Affordable Housing. The position involves serving as a resource person and case manager to support families involved in the Permanency Services program. Responsibilities include establishing and maintaining professional and supportive relationships with youth, resource families, and birth families, referring personnel, treatment providers, and others involved in the lives of children and youth. The role requires meeting regularly in the youth's home to provide skill development and ongoing case management services to coordinate the treatment planning and services provided according to the Treatment Plan and clinical direction. Additionally, the position involves maintaining regular contact with referring personnel and attending and testifying at court proceedings involving children and youth in care.

Requirements

  • A bachelor's degree in social service or a related field of study normally required.
  • A Master's Degree preferred.
  • A minimum of two (2) years related experience normally required.

Responsibilities
