The Recuperative Care Program (RCC) is a medical respite shelter program for single adults who are discharged from a hospital, emergency room, or other health facility and are experiencing homelessness while following an ongoing care/treatment plan. The RCC is hiring for a Case Manager II, which is the journey level class and is expected to work with a minimum amount of supervision. Incumbents are required to use assessment, interviewing, and counseling skills to perform moderate to complex casework services.
