About The Position

People Incorporated is seeking a Case Manager II to join their Waiver Case Management Dakota County team. This role involves working with clients diagnosed with chronic mental illness, supporting them in connecting with resources to live as independently as possible, and helping them develop self-awareness for positive future planning through frequent community meetings. Case Managers develop long-term client relationships, operate autonomously, and create their own schedules. The Waiver Case Management role emphasizes behind-the-scenes work supporting client needs over direct client meeting time. Requirements

  • Valid driver’s license
  • excellent driving record

  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions
  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions

Responsibilities

  • Must be able to have onsite presence within the Twin Cities, Minnesota.
  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
