AWARE is seeking an SDMI Waiver Lead Case Manager to provide high-quality care to individuals in the community. Requirements

  • A BS/BA Degree in a social work-related field and minimum of two (2) years of direct work experience in adult mental health case management.
  • Ability to develop and implement individual treatment plans with the knowledge of behavioral, clinical, and mental health interventions, skill building techniques, and understanding of the emotional and behavioral needs of assigned clients.
  • One (1) year experience in the human service field.
  • Interpersonal oral and written communications skills to work as a team member, to make observations and to report them accurately, to learn behavioral, clinical, and mental health care methods, and to follow oral and written instructions and can facilitate meetings.

Responsibilities

  • Caseload and/or billing expectations and requirements must be met on a monthly, quarterly, and yearly basis.
  • Maintain a consumer caseload established with designated supervisor. Actual caseload composition will be determined by the intensity of his/her caseload, and the specific service structure in the region.
  • Provides coordinated, comprehensive development of a Person-Centered Recovery Plan (PCRP) and monitors that plan as required by established rules.
  • Assists with gathering information on service availability and engagement statewide to support the service mapping process.
  • Assists with training and orientation of employees within the SDMI Waiver Case Management program.
  • Engages in community outreach and education as appropriate.
  • Supports the SDMI Waiver enrollment process as needed, through assessment of needs and confirmation of service availability for waitlist members approved for services.
  • Assists the member's entry into services and monitors that the needed services and benefits are received.
  • Completes chart audits as directed and required by the program.
  • Supports with service plan reviews and feedback to ensure compliance and alignment with policy.
  • May assess client, family, and environments, including transporting persons in corporate and/or personal vehicles and conducting regular home visits.
  • Follows State of Montana and AWARE rules, policies, procedures, and guidelines as they apply to the service.
  • Prepares reports, applications, and supporting documentation as requested by the supervisor within the timelines established by that supervisor.
  • Attend workshops, conferences, and meetings and may participate in committees as agreed upon with the supervisor, presents information to other Case Managers and A.W.A.R.E., Inc. employees, as requested.
  • Recognizes and utilizes AWARE’s Core Values, Unconditional Care Principles, and wrap around philosophy focused on client and family engagement.
  • Attends and participates in mandatory training, in-services, staff meetings, and committees as assigned and noted in AWARE’s Employee Handbook.
  • Attends and participates in mandatory training, in-services, abuse prevention/mandatory reporting training, staff meetings, and Corporate Committees as assigned.
  • Must demonstrate – on a consistent basis – the willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals (billing, case load number maintenance, weekly) must be met on a monthly basis.
  • Records notes, billing, assessments, referrals, and other data entry assignments via AWARE’s Electronic Healthcare Record System, Plan of Care - - being sensitive to age, gender, disability, sexual orientation/identity, racial, bi-sexual, transgendered, and ethnic differences of individual served.
  • Perform other duties as assigned.
