Under the direction and guidance of the Program Manager, you will be responsible for providing comprehensive, strength-based case management services to homeless and/or marginally housed transitional aged youth (ages 18-24). You will conduct a comprehensive evaluation of bio-psychosocial, emotional, educational, vocational and functional needs of young people; develop an individualized service plan in coordination with Larkin Street Youth Services continuum of care as well as leverages relevant community resources as needed. The Case Manager II leads collaborative service planning and must have a thorough understanding of the stages of change and development. The Case Manager II must be comfortable providing services to clients outside of normal office settings, including but not limited to emergency shelter environments, congregate living housing placements, scattered site housing, and single room occupancy (SRO) placements throughout the city of San Francisco. This is a full-time position, eligible for full benefits.
