About The Position

Diakon is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Child, Family, and Community Ministries and Diakon Affordable Housing. The position involves providing a full range of in-depth social work services, including case management to client populations in accordance with state regulations and agency policies. The role employs a range of social work interventions with client families to facilitate the accomplishments of goals listed in the Service Plan. Responsibilities include making initial case assessments and ongoing assessments, as well as providing case management and face-to-face services as described in the Service Plan to meet stated client goals.

Requirements

  • A bachelor's degree in social work, BSW, normally required.
  • A minimum of two (2) years' experience in a related child welfare or human service position normally required.
  • Experience in social work or case management is essential.

Responsibilities

  • Provide full range of in-depth social work services, including case management to client population in accordance with state regulations and agency policies.
  • Employ a range of social work interventions with client families to facilitate accomplishments of goals listed in the Service Plan.
  • Make initial case assessment and ongoing assessments.
  • Provide case management and face-to-face services as described in Service Plan, to meet stated client goals.
