About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Based on the Assertive Community Treatment (ACT) model, STRIDES of Alameda County is an intensive community support service for 120 individuals with severe mental illness (SMI), between the ages of 18-62, who are considered to be at great risk for psychiatric hospitalization.

Requirements

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc. (weekly mileage reimbursement at the IRS rate)
