Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare AdROC provides case management services to adults, aged 18 and older, who utilize emergency rooms, inpatient settings, and crisis services to regularly manage their mental health and substance use needs. Through a "whatever it takes" outreach and engagement approach, these programs help reduce the need for inpatient and emergency room care while improving mental health outcomes for the members served by connecting members to supports and resources available within their community. These programs work specifically with Alameda County residents who are not yet connected to county healthcare services. Program staff will provide linkages, supports, and resources to help community members remain in the least-restrictive, most self-sufficient, and recovery-oriented settings possible. The goals of these programs include reducing inpatient and emergency room care and improving mental health outcomes in Alameda County.
