About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare’s ARISE (Adult Recovery in StrengthâBased Environment) program serves 200 adult member high fidelity, strengths model Full Service Partnership (FSP) who meet criteria for the most intensive level of outpatient care and focus on three key populations with serious and persistent mental illness: individuals who are homeless, justiceâinvolved, and have coâoccurring substance use issues.

Requirements

High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
Willingness to use the company vehicle to drive clients to appointments and groups, etc.
