Requirements

Bachelor’s degree in related field or commensurate experiences.
4 Years of Case Management Experience

Responsibilities

Ability to handle complex case management issues and problems
Ability to identify and implement creative solutions and solve complex, challenging and changing situations.
Assesses treatment needs and develops care plans with mutually established goals for the people we serve
Monitors and assesses client progress by conducting routine evaluations.
Collects and analyzes treatment/results data, maintaining accurate client records.
Communicates and coordinates treatment plans to the client and the client’s guardian/family.
Addresses client needs, serving as an advocate when problems and issues arise.
