Requirements

• Bachelor’s degree in related field or commensurate experiences.
• 4 Years of Case Management Experience

Responsibilities

• Ability to handle complex case management issues and problems
• Ability to identify and implement creative solutions and solve complex, challenging and changing situations.
• Assesses treatment needs and develops care plans with mutually established goals for the people we serve Monitors and assesses client progress by conducting routine evaluations.
• Collects and analyzes treatment/results data, maintaining accurate client records.
• Communicates and coordinates treatment plans to the client and the client’s guardian/family.
• Addresses client needs, serving as an advocate when problems and issues arise.
