The Intensive Case Management Services (ICMS) Case Manager at Special Service for Groups, Inc. (HOPICS) is a full-time position responsible for providing comprehensive case management services to clients who are homeless or chronically homeless. Under the supervision of the ICMS Program Manager, the Case Manager will engage with a caseload of approximately 20 clients, ensuring they receive the necessary support to transition from homelessness to permanent housing. This role requires a deep understanding of the mission, vision, and core values of HOPICS, as well as the ability to articulate these to clients and stakeholders. The Case Manager will be tasked with developing individualized service plans tailored to each client's needs, which may include housing and rental assistance, health and mental health services, crisis management, and life skills training. Regular face-to-face meetings with clients are essential, with a minimum requirement of once a month, and the majority of services (90%) will be conducted in the field. The Case Manager will also be responsible for conducting assessments, updating service plans, and ensuring compliance with various regulations, including HIPAA and funding requirements. In addition to direct client services, the Case Manager will collaborate with various stakeholders, including DHS staff, property management, and community service providers, to facilitate access to resources and support. The role may require attendance at meetings and training sessions, as well as occasional evening and weekend work. The Case Manager must maintain accurate records of client interactions and services provided, ensuring that all documentation meets auditing standards. This position is critical in helping clients navigate the complexities of securing housing and accessing necessary services, ultimately contributing to their long-term stability and success.
