Seeking a full-time Housing Case Manager to join our CalAIM - Community Supports team. The Housing Case Manager will offer case management services to Justice Involved (JI) individuals transitioning from homelessness to temporary or permanent housing or assist those formerly homeless in retaining housing. The ideal candidate will have a client-centered approach and provide excellent customer service, addressing the challenges faced by homeless individuals with medical or behavioral health issues as they transition to permanent housing. The ideal candidate will possess extensive knowledge of local resources and community agencies and demonstrate strong communication and problem-solving skills to effectively address members' needs. Experience conducting in-home visits, visiting homeless encampments, and interacting within correctional facilities is essential. Preference will be given to candidates with personal experience of the justice system or a proven record of working with individuals involved with the justice system. Duties include but are not limited to serving as the main contact and advocate for justice involved individuals. This role includes outreach, housing assessments, creating Housing Support Plans, setting goals, addressing barriers, providing rental assistance, linking to health and support services, benefit establishment, transportation, crisis management, eviction prevention, client and landlord education, and housing location services. The position also requires maintaining active engagement and interaction with program-enrolled members to ensure proper coordination of care. Candidate must have a minimum of 1 year experience working as a Housing Case Manager, as well as a background in working with individuals who have mental health conditions and substance use disorders. Candidates must have a flexible schedule that includes late evenings and weekends.
