About The Position

- The Case Manager I function as an advocate and support person to those individuals assigned to their assigned program and links them to appropriate services and monitors the progress or lack of progress by the individual and his or her family. The Case Manager I will work closely with the individual, the individual’s family, LifeStream staff, and other community organizations to meet their needs and to allow the individual to live as independently as possible within the community.

Requirements

  • Bachelor’s degree with major course work in the human services field required
  • Minimum of 1 year of experience in case management or a related behavioral health field.
  • Experience working with diverse populations, including individuals with mental health and substance use disorders.
  • Familiarity with community resources and social services.
  • Strong interpersonal and communication skills, with the ability to engage effectively with clients and stakeholders.
  • Excellent organizational and time management skills, with the ability to handle multiple cases simultaneously.
  • Proficiency in using case management software and Microsoft Office Suite.
  • Ability to work independently and collaboratively in a team environment.
  • Strong problem-solving skills and the ability to make informed decisions in a timely manner.
  • Knowledge of relevant laws, regulations, and ethical standards in behavioral health services.

Responsibilities

  • Conduct thorough assessments of clients' behavioral health needs and document findings accurately.
  • Develop and implement individualized service plans tailored to each client's specific needs and goals.
  • Coordinate and facilitate access to necessary services and resources, including medical, social, educational, and vocational support.
  • Monitor and evaluate clients' progress towards their goals, adjusting service plans as needed.
  • Advocate for clients' needs and rights within the community and with service providers.
  • Maintain detailed and confidential client records in compliance with organizational and legal standards.
  • Collaborate with a multidisciplinary team to ensure a holistic approach to client care.
  • Provide crisis intervention and support as needed to ensure client safety and stability.
  • Participate in ongoing training and professional development to stay current with best practices in case management and behavioral health.
