The Case Manager I provides supportive, community-based services to clients living with mental health and co-occurring substance use needs as part of FSP team. This is a field-based position. Under general supervision, this position assists clients in accessing and navigating behavioral health, medical, housing, and social service systems to promote stability, wellness, and independence. The Case Manager I is responsible for delivering strengths-based, culturally responsive services, supporting treatment goals established by clinical staff, and ensuring continuity of care through ongoing coordination and communication with internal and external providers. This role includes conducting outreach and engagement, gathering psychosocial information, monitoring client progress, and documenting services in compliance with agency, county, and state standards. The Case Manager I work collaboratively with clinicians, peers, and multidisciplinary partners to address barriers related to housing, benefits, transportation, health, and daily living needs. Ideal candidates demonstrate strong organizational skills, empathy, professionalism, and the ability to build rapport with diverse populations.
