Our Housing Programs provide safe, permanent supportive housing paired with individualized services that help individuals and families build stability and move toward long-term independence. Case Managers play a key role in supporting residents as they navigate challenges, strengthen life skills, and maintain housing, with guidance and support from an experienced team. The Role As a Case Manager I, you play a key role in supporting individuals and families living in permanent supportive housing. The work centers on building meaningful relationships, helping clients identify and move toward personal goals, and connecting them with resources that support long-term housing stability. In partnership with a multidisciplinary team, this role includes assessing client needs, developing individualized service plans, and providing ongoing guidance and advocacy, with support and direction from supervisors and experienced team members. It is a great opportunity for someone early in their case management career who is compassionate, organized, and committed to making a direct impact in the lives of others.
