About The Position

Seeking a full-time Housing Case Manager to join our CalAIM – Community Supports team. The Housing Case Manager will offer case management services to Justice Involved (JI) individuals transitioning from homelessness to temporary or permanent housing or assist those formerly homeless in retaining housing. The ideal candidate will have a client-centered approach and provide excellent customer service, addressing the challenges faced by homeless individuals with medical or behavioral health issues as they transition to permanent housing. The ideal candidate will possess extensive knowledge of local resources and community agencies and demonstrate strong communication and problem-solving skills to effectively address members' needs. Experience conducting in-home visits, visiting homeless encampments, and interacting within correctional facilities is essential. Preference will be given to candidates with personal experience of the justice system or a proven record of working with individuals involved with the justice system.

Requirements

  • Minimum of 1 year experience working as a Housing Case Manager.
  • Background in working with individuals who have mental health conditions and substance use disorders.
  • Flexible schedule that includes late evenings and weekends.
  • Valid California driver's license and insurance.

Nice To Haves

  • Bilingual English/Spanish preferred.

  • Personal experience of the justice system.

Responsibilities

  • Serve as the main contact and advocate for justice involved individuals.
  • Conduct outreach and housing assessments.
  • Create Housing Support Plans and set goals.
  • Address barriers and provide rental assistance.
  • Link clients to health and support services.
  • Establish benefits and provide transportation.
  • Manage crises and prevent eviction.
  • Educate clients and landlords about housing.
  • Provide housing location services.
  • Maintain active engagement and interaction with program-enrolled members.
