About The Position

This position is located in Bartlesville, Oklahoma. The Case Manager I/II role involves providing services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The position requires obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, and serving as QIDP while providing monthly monitoring of the program. The role is under the direction of a case management supervisor and requires compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.

Requirements

  • For Case Manager - Level I: Bachelor's degree in a human service field and one year of general experience working directly with individuals with developmental and/or intellectual disabilities, or a valid permanent Oklahoma Nursing license and one year of general experience.
  • For Case Manager - Level II: Bachelor's degree in a human service field and one year of professional experience working directly with individuals with developmental and/or intellectual disabilities, or a Bachelor's degree and one year of professional experience in related fields plus one year of general experience, or a valid permanent Oklahoma Nursing license with one year of professional experience and one year of professional nursing experience.
