Case Managers provide assessment, case management, crisis intervention and integrated care coordination services to people with serious mental illness and co-occurring substance use disorders. Position includes, but is not limited to: assessment, person-centered planning, referrals, assisting with benefit procurement, crisis intervention, advocacy, skills training, support with taking medications, care coordination with healthcare providers and connecting people with community resources. Case Managers may also help to facilitate groups. Services are provided in both the office and community. . Case manager must function well as part of a collaborative team and have good knowledge of community resources. Our case managers work under the supervision of the Mental Health Program Director and Program Coordinator. Requirements

  • Case managers must have experience treating adults with serious mental health needs and with co-occurring substance use needs too.
  • Some evening hours may be required.
  • A valid MI driver's license is required.
  • Case Manager III - Master's degree with Michigan licensure, either LMSW, LLP or LPC.
  • Case Manager II - Master's degree with Michigan licensure, either LLMSW, TLLP, LLPC and the ability to obtain full licensure within 2 years of hire. Promotion to Case Manager III upon receipt of full license.
  • Case Manager I: Bachelor's degree in a human services-related field OR bachelor's degree in social work. If degree in social work, licensure as LBSW or LLBSW is required. If licensed at LLBSW level, the individual must obtain full LBSW within two years of hir e.
