Conducts intake interviews to assess the needs of the client. Once needs have been determined, plans a course of action coordinating with variety of Sussex County outreach providers for food, clothing and shelter to meet the client's needs. Obtains medical and employment history and sets up medical appointments and mental health or family counseling as required. Arranges and observes appropriate boundaries with clients. Observes client confidentiality and HIPAA protocols. Ensures resources are used by eligible households who are homeless or at the risk of homeless. Maintains and documents regular contact with clients: health center visits, telephone contact, home visits. Provides transportation, as required, to health center, or other required service agencies. Attends appropriate local, regional and national meetings on homeless issues to seek out new current information. Prepares and presents training and educational programs and publicity materials. Participates in team efforts to make La Red a welcoming environment for clients utilizing our services including persons participating in our homeless case management. Creates written statistical technical reports for review to the Behavioral Health Administrator Performs other duties and responsibilities as assigned.
