Job Summary : Responsible for providing person-centered case management to reentry parents through a collaborative individualized case plan. Promote services to participants to build parental engagement, coparenting skills, and economic stability. For incarcerated participants, engage in person-centered transitional planning for successful reentry that bridges pre and post release programming. Support participants in obtaining identification documents. Support relevant reentry parent workshops and participant groups. Connects participants to community services, including health, mental health, housing, treatment, recovery support, employment, and education services, as per their individualized case plan. Regularly follows-up with participants and service providers are required, updating the reentry plan as needed.
