Job Summary : Responsible for providing person-centered case management to reentry parents through a collaborative individualized case plan. Promote services to participants to build parental engagement, coparenting skills, and economic stability. For incarcerated participants, engage in person-centered transitional planning for successful reentry that bridges pre and post release programming. Support participants in obtaining identification documents. Support relevant reentry parent workshops and participant groups. Connects participants to community services, including health, mental health, housing, treatment, recovery support, employment, and education services, as per their individualized case plan. Regularly follows-up with participants and service providers are required, updating the reentry plan as needed.

Requirements

• Demonstrated experience in case management, required
• Excellent communication, writing and computer skills, required
• valid NYS Driver’s License, access to registered and insured vehicle, and transporting participants in own vehicle, required
• occasional travel to out of town/state conferences and meetings, required
• ability to gain clearance to OCCF, required

• BA/BS in public administration, health, social work, criminal justice, preferred.

Responsibilities

• Develop person-centered individualized case plans with each participant focusing on personal and family needs, housing, finances, employment, physical & mental health, substance use recovery and treatment.
• Meet with participants consistently, both pre and post release, as you follow the goals outlines in the individualized career plan.
• Engage partner resources as relevant through warm handoffs.
• Document participant progress and performance through detailed record keeping and case notes.
• Distribute participant incentives in accordance with program policies and procedures.
• Provide coaching and feedback to participants around engagement in programs, workshops, and training.
• Address barriers to ensure workshop and program completion.
• Receive training in child abuse and domestic violence prevention.
• Promote or refer participants to relevant resources.
• Follow established procedures for reporting and intervention.
• Assist with in-facility recruitment and enrollment and support groups while adhering to curriculum fidelity as needed.
• Coordinate case management and reentry services with partner facility staff, as needed, taking into account schedules and participant movements.
• Transport participants from the jail on the day of release to the CCA office to review and support participant goals outlines in the individualized case plan.
• Support participants in completing resumes, applications, and job searches to obtain and retain employment.
• Support parent support and job retention groups, working cooperatively with the team to ensure participant participation.
• Provide crisis intervention - including quick assessment of the nature of the crisis and identification of appropriate methods of intervention.
• Comply with all policies and procedures of partnering training sites, such as Onondaga County Correctional Facility (OCCF).
• Develop strong working relationships with OCCF staff.
• Participate in staff meetings, case conferencing and program evaluations.
• Participate in in-service and outside training, as approved by supervisor and/or as required.
• Perform other duties as assigned by supervisor.
