About The Position

The Case Manager provides direct support to clients at UBH’s Culver City Crisis Housing Shelter, helping families transition out of homelessness. This role combines case management with program coordination, ensuring clients receive trauma-informed, individualized services while maintaining strong partnerships with internal teams and community providers.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
  • 3+ years’ experience in homeless services (supervisory experience a plus).
  • Knowledge of Trauma-Informed Care, Housing First, Motivational Interviewing, Harm Reduction.
  • Strong crisis intervention, organizational, and communication skills.
  • Proficiency in HMIS and Microsoft Office; bilingual (English/Spanish) preferred.
  • Employment eligibility verification, reliable transportation, valid CA driver’s license, and insurance.
  • Background check, TB test, CPR/First Aid certification.

Responsibilities

  • Maintain a caseload, conduct assessments, and create individualized service plans.
  • Provide crisis intervention, counseling, and emergency response.
  • Document client progress and ensure compliance with agency and funder requirements.
  • Connect clients to internal and external services and monitor progress toward goals.
  • Assist clients in securing permanent housing within targeted timelines.
  • Guide clients through applications, landlord communication, and move-in preparation.
  • Build relationships with property managers and maintain a housing resource inventory.
  • Collaborate with Resident Manager, Employment Specialist, and Clinical staff to align services.
  • Participate in weekly coordination meetings and support daily program operations.
  • Ensure shelter rules, safety standards, and incident response procedures are followed.
  • Develop and maintain referral networks with local service providers.
  • Represent UBH at community meetings and coordinate with external stakeholders.
  • Maintain accurate case files, service plans, and reports.
  • Support program evaluation and continuous improvement initiatives.
  • Ensure compliance with funding requirements and program policies.
