About The Position

We support community members who are at most risk for long-term psychiatric hospitalizations. This multidisciplinary team provides a wrap-around approach to support adults with severe and persistent mental illness. We meet clients in their homes and neighborhoods, with the goal of helping them to maintain stability and be successful within the community.

Requirements

  • Education: Bachelor’s degree in Social Work, psychology or related human service
  • Experience: At least one year experience with individuals with a severe and persistent mental illness. Excellent clinical skills with a recovery orientation.
  • Must be 21 years of age.
  • Negative Drug Screening prior to hire and throughout employment.
  • Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually, along with DCF Level II prior to hire and as required by DCF
  • Clear Florida Driver’s License Motor Vehicle checks prior to hire and annually with acceptable driving record throughout employment. Must possesses reliable vehicle, valid registration, and valid vehicle insurance.

Responsibilities

  • Collaborate with clients with severe and persistent mental illness to develop realistic, effective, and measurable treatment plan goals and objectives, with appropriate time frames
  • Use of solution-oriented intervention strategies and principles of recovery, as well as respectful and competency-based language and interaction with clients, their families, and other support networks
  • Ensure that clients have basic needs met, including housing, legal advocacy, medical/dental services, financial support, activities of daily living skills, vocational assistance, leisure activities, substance abuse treatment and other services on their individualized treatment plan
  • Assist clients in managing their physical and mental health, connecting them to services and benefits, and overcoming barriers to these services and benefits.
  • Complete and maintain documentation in a timely and accurate manner, within agency and program standards
  • Participate in 24/7 on-call response system to provide crisis intervention as requested
  • Participate in daily team meetings, individual and group supervision, and organizational meetings, updating program staff on client progress and concerns
  • Coordinate with client’s family members and other community supports to ensure needs are met
  • Deliver medications to clients as needed
  • Maintains productivity level in accordance with agency standards
  • Demonstrate consistent excellence in clinical skills
  • Accomplishes individual goals
  • Other job duties as assigned
