About The Position

The Case Manager Facilitator "Wraparound Facilitator" provides comprehensive support and coordination for children and their families who are enrolled in the SED Waiver. The ultimate goal is to ensure that youth with complex behavioral health needs receive individualized, community-based care that promotes their overall well-being and success in various life domains. Additionally, the facilitator works closely with insurance company personnel to coordinate, develop, and maintain these care plans

Requirements

  • Bachelor’s degree in human services preferred or with other significant relevant human services experience. Alternatively, four years of education/experience in the human services field.
  • Must be at least 21 years of age
  • Proficient in the English language
  • Able to pass pre-employment background and drug screening
  • Have completed or is able to complete within the first 60 days of employment SED waiver, case management and crisis training as required by the State of Kansas.
  • Strong organizational and time management skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a team.
  • Proficient in the use of computers and electronic health record systems.
  • Required to maintain professional boundaries and confidentiality.
  • Individuals must be physically capable of operating a company vehicle safely, possess a valid driver’s license, and have an acceptable driving record.

Nice To Haves

  • Experience working with individuals with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), or Substance Use Disorders (SUD) is preferred.

Responsibilities

  • Collaborate and coordinate with families, guardians, foster care agencies, schools, and any other collateral contacts as identified by the family.
  • Facilitate regular meetings with the youth's treatment team, including mental health professionals, educators, probation officers, and other community partners, to ensure coordinated and integrated services.
  • Identify client referrals and bring the team together to complete initial SED Waiver paperwork.
  • Work closely with client insurance company personnel in coordinating, developing, and maintaining the plan of care.
  • Train and educate other members about waiver services, including professionals outside of the agency.
  • Follow Medicaid guidelines on the delivery and documentation of services.
  • Maintain accurate and up-to-date records of the youth's progress, interventions, and service utilization in compliance with regulatory standards and confidentiality requirements.
  • Monitor the implementation of the care plan and regularly evaluate its effectiveness in achieving desired outcomes.
  • Reassemble the wraparound team for subsequent reviews or revisions, at least every 90 days and annually to assess the level of care.
  • Document any changes in the youth's circumstances or needs and adjust the care plan accordingly in collaboration with the treatment team.
  • Manage a small client caseload by providing ongoing case management and waiver services.
  • Other duties as assigned.
