About The Position

Helio Health is seeking a Case Manager to support its mission of promoting recovery from substance use and mental health disorders. This full-time position is essential for managing and coordinating services for patients in the Elements of CNY Residential Program located in Liverpool, NY. The role involves a variety of responsibilities including case management, crisis intervention, and collaboration with community providers.

Requirements

• Associate's degree in a related human services field required, Bachelor's preferred.
• Two years of experience working with behavioral health and/or substance abuse issues.
• Valid NYS Driver's License preferred.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including screening, intake, and orientation.
• Provides referrals and counseling to patients/residents.
• Conducts crisis intervention and patient/resident education.
• Manages case documentation and reporting.
• Consults with other professionals and coordinates care with community providers.
• Plans for patient discharge and trains staff, volunteers, and interns.
• Performs quality assurance activities and monitors service provision.
• Evaluates programs to identify strengths and weaknesses.
• Assists in problem resolution and implements approved program modifications.
• Participates in program administrative and clinical committees as assigned.
• Represents the agency to the community and performs other duties as assigned by supervisor.
