About The Position

The Case Manager serves as a support coordinator, ensuring that services to individuals with developmental disabilities and intellectual disabilities are planned, individualized, and person-centered. The Case Manager provides services that may include assistance to individuals and their family members in accessing needed services that are responsive to the individual’s needs.

Requirements

• Bachelor’s degree from an accredited college or university in an applicable human services field.
• A minimum of one (1) year of directly related work experience providing case management to individuals with developmental disabilities.

Responsibilities

• Serve as a support coordinator for individuals with developmental and intellectual disabilities.
• Assist individuals and their family members in accessing needed services.
• Coordinate the provision of services by public and private providers.
• Engage and sustain ongoing relationships with individuals receiving services.
• Complete DBHDS core competency-based curriculum within 30 days of hire.
• Participate in and successfully pass behavioral intervention training.
• Drive and transport clients as needed to various locations.
