About The Position

SSM Health Day Institute is seeking a Case Manager to work in our specialized outpatient rehabilitation program. We have nine centers across the St. Louis Metro area serving individuals recovering from a traumatic injury or illness who no longer require 24-hour nursing or acute rehabilitative care. We take a comprehensive, multi-disciplinary approach to build skills and maximize independence. Requirements

• Professional Healthcare Degree in either Nursing, Social Work, Licensed PT, OT, or SLP.
• One year rehab case management experience (preferred).

Responsibilities

• Monitors and assesses patient plan of care from pre-admission to post-discharge.
• Assists patient/family in goal setting to maximize patient’s independence in both home and community.
• Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.
