About The Position

BASIC PURPOSE: To provide case manager services to persons with severe and persistent mental illness in the CSP program which allows them to live successfully in the community and grow their recovery. ESSENTIAL DUTIES & RESPONSIBILITIES: Provide assessment and treatment planning for persons with severe and persistent mental illness, including assessment, recovery and crisis planning at intake and every six months or as otherwise indicated. Ensure appropriate discharge referrals are made and documented in the discharge summary in a timely manner. Implement services as needed to assist the client in meeting their recovery plan goal. As determined needed by the assessment and described in the recovery plan. Client services are directed by a recovery oriented, strengths based approach. Assist the client in managing psychiatric symptoms, medical issues that co-occur with mental health issues and substance use issues. Provide symptom monitoring/management and education of symptom management tools, crisis intervention services, and medication education and monitoring as directed by the Medical Director and Clinical Coordinator. Ensure client’s needs are met related to safety including ensuring the client has access to appropriate housing, food and other health and safety issues needed to maintain independent living. This may include providing rep payee services as needed. Attend all scheduled treatment meetings including individual supervision and engage in treatment discussions with both internal and external treatment providers in a timely manner. All client service related paperwork is completed in a thorough and timely manner and as required by DHS 63. Provide case management services as necessary to ensure that the client maintains access to services needed to help them reach their recovery plan goal and maintain their psychiatric and physical health. Case management also includes engagement and coordination with family/support system as client allows. Provide after-hours crisis services to CSP clients by phone or as needed face to face.

Requirements

  • Required is a bachelor’s degree in a behavioral science or a related field with 1,000 hours of supervised post-degree clinical experience with chronically mentally ill persons, or a bachelor’s degree in a field other than behavioral sciences with 2,000 hours of supervised post degree clinical experience with persons with chronic mental illness.
  • Familiarity with the human service delivery system, DHS 63, recovery principles, and appropriate ethics and boundaries.
  • Ability to use Microsoft Office programs, e-mail, scanner, and FAX machine, as well as enterprise software.
  • Driver’s license and ability to use own vehicle to provide client transportation.

Nice To Haves

  • Preferred is a master’s degree in social work, clinical psychology or psychiatric mental health nursing or having met equivalent educational requirements with 3,000 hours of supervised clinical experience working with persons with severe and persistent mental illness outside of a CSP program or 1,500 hours of supervised clinical experience working within a CSP program is preferred.
