This is an opportunity to support the mental health of your clients. You will be responsible for coordinating and providing comprehensive care and support for your clients .

Requirements

• High school diploma with 3-5 years of experience or Bachelor’s degree in mental health related field.
• A driver’s license with 3-points or less and access to an insured vehicle.
• This role may have on-call hours.

Responsibilities

• Coordinating services for assigned clients , including psychiatric and medical care, housing, entitlements, etc.
• Developing individualized service plans with clients based on their goals.
• Providing ongoing and supportive assistance to clients.
