Case Manager-Rhode Island-Local Travel Required What You'll Do: Manage a caseload and conduct outreach (telephonic & community-based) Complete assessments in-home or in the community Coordinate medical services and referrals Assist participants in accessing social services and making appointments Conduct off-site visits to homes, hospitals, and community agencies Collaborate with interdisciplinary teams and contribute to program improvements Maintain accurate documentation in EMR Participate in outreach and marketing activities
Requirements
- Bachelor's degree or equivalent experience (Community Health Worker certification accepted)
- Experience with adults with disabilities, chronic illness, behavioral health, or substance use
- Strong knowledge of Rhode Island community resources
- Excellent communication and cultural sensitivity
- Ability to work independently and as part of a team
- CPR/BLS certification (American Heart Association or Red Cross)
- Commitment to confidentiality and HIPAA compliance
- Flexible scheduling to meet participant needs
- Work in a collaborative, mission-driven environment
