Case Manager-Rhode Island-Local Travel Required What You'll Do: Manage a caseload and conduct outreach (telephonic & community-based) Complete assessments in-home or in the community Coordinate medical services and referrals Assist participants in accessing social services and making appointments Conduct off-site visits to homes, hospitals, and community agencies Collaborate with interdisciplinary teams and contribute to program improvements Maintain accurate documentation in EMR Participate in outreach and marketing activities What you Bring: Bachelor's degree or equivalent experience (Community Health Worker certification accepted) Experience with adults with disabilities, chronic illness, behavioral health, or substance use Strong knowledge of Rhode Island community resources Excellent communication and cultural sensitivity Ability to work independently and as part of a team CPR/BLS certification (American Heart Association or Red Cross) Commitment to confidentiality and HIPAA compliance Why You'll Love This Role: Make a real difference in people's lives Flexible scheduling to meet participant needs Work in a collaborative, mission-driven environment
