About The Position

The CBS (Community Based Services) Case Manager will be responsible for providing comprehensive face-to-face case management services to youth within the home, school, or community settings. The Case Manager will work collaboratively with a multidisciplinary team to support the mental health and well-being of youth clients, ensuring they receive appropriate services and resources to address their wellbeing. This role involves planning, coordination, monitoring, and advocacy to help clients achieve their treatment goals and improve their quality of life.

Requirements

  • Bachelor’s degree in human services preferred or with other significant relevant human services experience. Alternatively, four years of education/experience in the human services field.
  • Must be at least 21 years of age
  • Proficient in the English language
  • Able to pass pre-employment background and drug screening
  • Have completed or is able to complete within the first 60 days of employment case management and crisis training as required by the State of Kansas.
  • Strong organizational and time management skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a team.
  • Proficient in the use of computers and electronic health record systems.
  • Required to maintain professional boundaries and confidentiality.
  • Individuals must be physically capable of operating a company vehicle safely, possess a valid driver’s license, and have an acceptable driving record.

Nice To Haves

  • Experience working with individuals with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), or Substance Use Disorders (SUD) is preferred.

Responsibilities

  • Provide direct services in clients' homes, schools, and community settings.
  • Develop and implement individualized service plans in collaboration with clients and their families.
  • Advocate for clients' needs, assisting them in accessing community resources and services.
  • Connect clients to crisis intervention services and assist in safety planning alongside the crisis team as needed.
  • Support clients in developing and enhancing life skills, social skills, and coping strategies.
  • Work closely with community partners, healthcare providers, and educators.
  • Participate in treatment team meetings to discuss client progress and adjust service plans as scheduled.
  • Coordinate with other internal and external service providers to ensure comprehensive care.
  • Maintain professional relationships with all partners and agencies involved in clients' care.
  • Complete all required client service documentation accurately and promptly.
  • Maintain client confidentiality and adhere to all ethical and legal standards.
  • Document all services provided, including client interactions, referrals, and follow-up activities.
  • Attend and participate in all required training sessions and professional development activities.
  • Stay informed about best practices and advancements in mental health and substance use disorder treatment.
  • Other duties as assigned.
