This position is responsible for carrying out the job duties of Children’s Mental Health Case Management. The case manager is responsible for assessing client needs, planning for service implementation, coordinating services on behalf of clients, and monitoring services for consistency and efficiency. Case managers are expected to interact professionally with internal and external community providers, perform related work tasks as required or assigned and comply with Minnesota Rule 79 in delivery of Children’s Mental Health Case Management services. Case managers may be assigned to perform job responsibilities in one or more than one of the counties under the jurisdiction of Minnesota Prairie County Alliance. Under the direct supervision of Director of Children’s Services - SPERO and assigned Minnesota Prairie County Alliance Child and Family Supervisor(s).
