Calm Waters Counseling is seeking a Case Manager to enhance client management quality and maximize satisfaction. The role involves assessing, planning, implementing, monitoring, and evaluating actions required to meet clients' health and human services needs in the community. The Case Manager will coordinate care, advocate for clients, and work collaboratively with medical teams and social services to ensure effective treatment and support.

  • Coordinate and provide safe, timely, effective, efficient, equitable, and client-centered care.
  • Handle case assignments, draft treatment plans, review case progress, and determine case closure.
  • Help clients achieve wellness and autonomy.
  • Facilitate multiple care aspects including case coordination and information sharing.
  • Advocate for clients regarding their clinical status and treatment options.
  • Develop effective working relationships and cooperate with the medical team throughout the case management process.
  • Coordinate care with social services, health, and governmental agencies.
  • Initiate regular interactions with clients to track their progress and ensure satisfaction.
  • Record case information, complete necessary forms accurately, and produce statistical reports.
  • Promote quality and cost-effective interventions and outcomes.
  • Assess and address motivational and psychosocial issues.
  • Adhere to professional standards as outlined by protocols, rules, and regulations in the Social Work Code of Ethics.
  • Proven working experience as a case manager.
  • Excellent knowledge of case management principles.
  • Effective communication skills.
  • Excellent organizational and time management skills.
  • Familiarity with professional and technical emerging knowledge.
  • Problem-solving skills and ability to multi-task.
  • Compassionate with teamwork skills.
  • Bachelor's degree in a related healthcare/mental health field.
  • 1 year of experience in case management (Preferred)
