Our clinical team provides compassionate, high-quality counseling and therapy to patients that are seeking recovery from Opioid Use Disorder. Your Responsibilities as a Case Manager: Provides quality care through developing, implementing, managing, and evaluating patient care plans; Collects clinical data, correlates this clinical data, and provides clinical input to the treatment team; Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted; May also correlate clinical data for the business office as required. Providing case management and care coordination services for patients in the clinic; Responsible for coordinating the continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements; May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed; Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team’s relationship within the facility or treatment milieu; and, Perform other duties as assigned. Check your eligibility here: HRSA Eligibility Qualifications Your Education, Skills and Qualifications: Bachelor’s degree in Social Work, Counseling or related Human Service field. 1-2 years of experience working in behavioral health setting. Experience with the population served by the clinic preferred. Previous experience in case management and/or utilization review preferred. Licenses/Certifications: CADC Licensure or certification for counseling required.
