About The Position

The position involves providing in-home and in-office assessments of the social and emotional needs of older adult clients and their families. This includes developing and implementing case plans for a full range of social services, offering counseling, providing information about available services, and advocating for clients' benefits. The role requires ongoing evaluation of client needs and collaboration with community agencies to ensure effective service delivery.

Requirements

  • Graduate of an Accredited College or University with B.S.W./or B.A./B.S. degree, or equivalent.
  • Good writing and record-keeping skills.
  • Ability to read, write, speak, and understand English.
  • Computer literate, with the ability to learn and use program-specific software.
  • Ability to travel to clients' homes using mass transit.
  • Knowledge of Russian, Spanish, Patois, Mandarin, or Cantonese is helpful.
  • Understanding of the roles of race, religion, ethnicity, sexual preference, and individual values in serving clients.

Responsibilities

  • Provision of In-Home and In-Office Assessment of the social and emotional needs of older adult clients and their families.
  • Development and implementation of a written case plan for social services.
  • Individual and family counseling and guidance.
  • Provision of information regarding physical, social, and psychological development.
  • Screening client eligibility and advocating for benefits.
  • Arranging direct provision of services such as transportation and homecare.
  • Evaluating clients' capacity to manage their affairs and protecting them from exploitation.
  • Disbursement of funds for goods and services.
  • Referral and cooperation with community agencies.
  • Ongoing reassessment of client needs and monitoring service efficacy.
  • Informing supervisors of crisis situations.
  • Representing the agency at community conferences and meetings.
  • Participating in training and unit meetings.
  • Completing required case records and reports.
