About The Position

MCC is seeking a dedicated Case Manager to provide hands-on, home-based support to families in our Early Head Start (EHS) program in Pottstown. The Case Manager will maintain a caseload of EHS clients, assist with program development and coordination, and offer direct supervision to a small group of Advocates. Requirements

  • CDA or higher degree from an accredited college or university in a related field of study.
  • 2+ years of experience providing direct services in a community outreach setting preferred.
  • Experience with community health issues.
  • Knowledge and experience in child development, pregnancy, breastfeeding, and early childhood education.
  • Experience working with maternal and child health issues preferred.
  • Experience with positive parent-child interactions or Early Head Start programs preferred.
  • Bilingual (Spanish, Indonesian, Burmese, if applicable).
  • Minimum of 1 year of supervisory experience.
  • Valid driver's license.

Responsibilities

  • Provide early learning-focused and curriculum-based home visits to pregnant women and parents of infants and toddlers.
  • Promote families' ability to support their health improvement, social empowerment, and child's development toward school readiness.
  • Assist with program development, coordination, and supervision.
  • Provide leadership in conducting outreach and follow-up.
  • Supervise up to four Advocates.
  • Plan weekly visits that include activities to support learning and development.
  • Reschedule any missed visits to provide a minimum of 4 visits each month and a minimum of 46 visits to each client throughout the program year.
  • Plan home visits and provide education on prenatal development and resources to pregnant women at least twice a month.
  • Utilize a professional, culturally sensitive, and respectful approach when visiting clients and their families.
  • Provide curriculum-based education sessions and resources for pregnant women and families with infants and/or toddlers.
  • Conduct client assessments to determine strengths and areas of need.
  • Work collaboratively with Coordinators to ensure timely scheduling and completion of health and/or education-related appointments.
  • Participate in the collection and distribution of supplements, diapers, formula, clothes, and donations for EHS clients.
  • Refer participants needing more intensive follow-up to appropriate resources.
  • Encourage parent participation in all EHS events.
  • Attend, facilitate, and actively participate in Site Meetings and Socializations.
  • Participate in outreach efforts to raise awareness and promote education of issues faced by EHS clientele.
  • Respond promptly to inter-agency communication using Child Plus and other computer systems.
  • Meet regularly with supervisor monthly to discuss clients' progress.
  • Observe punctuality, responsibility, and accountability regarding their daily schedule.
